Has anyone ever encountered an issue in Okta where a user is unable to login and all the logs say are "failure: NOT_Specified"?

Nope, but my first steps would be to try three things in this order:
1. Reset their behavior profile.
2. Reset MFA
3. Reset Password

check if the user exists with the same profile
ex: and
if he inputs just mike.alex on login it will fail

also check if there is some login policy that has “then access is denied” where the user is part of

The logs would show which policy triggered the access denied though, right?

I've also had people clear their browser cache and fix mysterious Okta login page errors (we're not using behavior profiles currenlty). 🤷

true, if a policy is to blame , it should be in the logs

not sure if that is the same error code you get

but I would first check the thing I mentioned initially .. check if the user exists twice somehow with the same first.lastname or something
